DAPP Participation Form

I wish to be considered for participation in the DePaul Admission Partnership Program (DAPP). I understand that my uploaded Unofficial Transcript will now be reviewed by a DePaul staff member to ensure that I meet the DAPP eligibility requirements. After the transcript review, I will be notified by DePaul and asked to schedule an official DAPP appointment if eligible.

Once I receive notice of my acceptance into DAPP, I understand that I must adhere to all DAPP requirements which include:

• Meet with my DePaul Transfer Counselor at least once each semester while I am enrolled at my community college.
• Maintain satisfactory academic progress at my community college to remain admissible to DePaul and my program of choice (2.0/4.0 cumulative GPA or 2.5/4.0 GPA for the College of Education).
• Maintain continuous enrollment of at least two courses per semester (not including summer) while at my community college.
• Submit an Unofficial Transcript to DePaul each semester I am enrolled at my community college and participating in DAPP.
• Apply and enroll at DePaul as a transfer student no sooner than one year (two semesters) or no later than three academic years after joining DAPP.

I agree to my community college and DePaul University sharing my academic records to facilitate the awarding of the associate degree and the transfer of credits between the two schools. By checking the box below, I am giving DePaul University and my community college permission to exchange information about my academic record.
